We would like to thank Dr Maffulli for additional insights into the treatment of patellofemoral pain syndrome (PFPS). In the commentary, Dr Maffulli raises the point that any interventions result in improvement in patients with PFPS. We contend that conservative management programs,1,2 which have reported high success rates, have been designed to address the specific needs of the patient with PFPS such as correcting muscle imbalance, patellar tracking, and foot alignment. There is such a myriad of conservative treatments that can be applied individually or in combinations that there is a sense of bewilderment among clinicians as to what methods may be most expedient and effective in reducing their patient's symptoms and regaining pain-free function....
